Demonstrate the ability of the e-health solution and overall care model to increase patient outcomes over the first 8 months post-rehabilitation discharge across the care continuum, as compared to that seen prior to the implementation of the e-health solution
Centralize access to education resources for persons with spinal cord injury, foster improved self-management skills among participants and enhance communication with their healthcare team
